- This list of commodities classifications will help you identify the proper supply/service codes to enter on the Bidder's Application Form (BAF)
- Small businesses: If you are a small Illinois business per Section 11 of the Bidders Application Form, you may qualify for the Small Business Set-Aside Program.
Federal Taxpayer ID number - W-9 Form [PDF, 72 KB] 
Employer Report Form [PDF, 994KB] (Required if you have 15+ employees)
Certificate of Authority for Out-of-State Vendors [PDF, 152KB] (Required if you are a non-Illinois company)

Step 2: Apply for certification with the Business Enterprise Program
Complete the Recognition Certification Affidavit [PDF, 246KB] , if you are currently certified by one of the following entities:
- Illinois Department of Transportation (IDOT)
- Chicago Minority Business Development Council (CMBDC)
- Womens Business Development Center (WBDC)
- City of Chicago
- Chicago Transportation Authority (CTA)
- METRA
- PACE
- Cook County
OR
Complete the BEP Schedule A Certification Affidavit [PDF, 160KB] ,
if you are not currently certified by one of the entities above.
To continue your annual BEP certification, complete the Business Enterprise Program Annual Certification Renewal [PDF, 102KB]
